The USTA Foundation Scholarship Program

This award is dedicated to Tracey Donnelly Maltby, Diane Donnelly Stone, and their father, Bud Donnelly, who all competed in Division I collegiate tennis while dealing with Type 1 Diabetes complications.

At the University of Iowa, Tracey played singles and doubles for four years while earning All-Big Ten recognition.

At Northwestern University, Diane was a three-time All-American and the 1987 NCAA Doubles champion. She will be inducted into the ITA Collegiate Tennis Hall of Fame in 2022.

At Western Michigan University, Bud played singles and doubles for four years while winning six individual Mid-American Conference championships.

Since 2002, Novo Nordisk, a worldwide healthcare organization that has produced ground-breaking diabetic medications for nearly a century, has served as the award’s title sponsor.

Dexcom, Cecelia Health, and Tandem Diabetes Care, Inc. also support the scholarship.

This grant is open to high school seniors and college students who have excelled in tennis and academics, have a strong sense of leadership and community service and have been affected by diabetes.

The USTA Foundation will give two (2) national winners $10,000 each, ten (10) regional winners $5,000 each, and two (2) NJTL program awards of $2,500 each out of a total scholarship fund of up to $75,000.

About The Scholarship

The USTA Foundation scholarship will be given to students with diabetes who are enrolling in a four-year program at a college or university.

They are designed to cover the cost of tuition, books, and other associated costs at the college or university the student chooses.

The college or university where the student is enrolled will receive payment for these scholarships immediately.

Overall Directives

The USTA Foundation focuses on helping young people with little resources. Seniors in high school are given scholarships by the USTA Foundation if they:

• Present evidence of academic success (3.0 minimum GPA)

• Engage in community or volunteer work

• Take part in youth tennis programs run by organizations like National Junior Tennis and Learning (NJTL)

• Prove they have a serious financial need (confirmed through a two-stage procedure using data from the Free Application for Federal Student Aid, or FAFSA).

• By mid-April 2023, top candidates will be chosen and invited via email to submit the extra materials listed below, which must be uploaded directly to the student’s application (by signing into their USTA Foundation Funding & Program Portal account):

  • The FAFSA’s Student Aid Report (SAR).
  • A recent image (.jpg format, no larger than 2 MB)
  • Two letters of recommendation, one of which must include the USTA Foundation Scholarship Recommendation Form (which can be downloaded from the USTA Foundation Funding & Program Portal) and the other of which must list the student’s accomplishments, areas for improvement, and any other relevant information.
  • One from a mentor or instructor
  • One from a tennis mentor or coach
  • The college or institution receives direct payment for every scholarship award.
  • Applicants may submit applications for any of the scholarships listed that they are qualified for; however, no one will be given more than one scholarship.
  • All submissions, including applications and supporting documents, become the property of the USTA Foundation and cannot be refunded.
  • The USTA Foundation will supply recipients with a formal acceptance form, which they must return after receiving an email confirming their acceptance.
  • No later than the first fall semester or term after notification of selection for the scholarship, each recipient of a USTA Foundation scholarship is expected to enroll as a full-time undergraduate student in accordance with the requirements set forth by his or her undergraduate institution.
  • Applicants are not eligible for a USTA Foundation scholarship if they are receiving full grant and aid scholarships in tennis from their respective schools.
  • Following notification of his or her selection, the grant recipient must submit a formal request for deferment if he or she is unable to enroll or use the award during the first fall semester or term. The USTA Foundation alone will evaluate and make decisions about each request on a case-by-case basis. A recipient will lose their eligibility for the scholarship if they don’t get the USTA Foundation’s written consent for a postponement.
  • For scholarships that are payable over a four-year period, recipients must complete a verification form attesting to their excellent standing in school. This form will be issued by the USTA Foundation. They must also give a copy of their college transcript.
  • Should a scholarship recipient decide to transfer to another college or university, a formal request and a recent transcript attesting to their excellent standing must be sent to the USTA Foundation. The USTA Foundation alone will evaluate and make decisions about each request on a case-by-case basis.
